STNA

Category: Full-Time and Part-Time

Position Summary Under the direct supervision of the nurse in charge, the State Tested Nursing Assistant performs basic nursing duties to provide for the comfort, safety, and hygiene needs of residents. The State Tested Nursing Assistant assists residents in maintaining and possibly increasing their level of independence whenever possible.

Requirements State Tested Nursing Assistant by State of Ohio or eligible for reciprocity. Must be willing to complete the Certified Dementia Practitioner course. Qualified candidates must possess a genuine heart for the aging population and an appreciate of a faith based environment.

Registered Nurse / Licensed Practical Nurse

Category: Full-Time, Part-Time, and PRN

Position Summary Registered Nurse/Licensed Practical Nurse is accountable for the delivery of safe and therapeutic resident care from admission through discharge in collaboration with the physician, family, and other members of the multi-disciplinary health care team. Delivery of quality nursing care is assured through utilization of the nursing process. Registered Nurse/Licensed Practical Nurse serves as a positive clinical role model for staff as well as a resource person and supports the philosophy, mission, and core values of St. Leonard.

Requirements Must be a Registered Nurse or Licensed Practical Nurse in the State of Ohio. Long-term care experience preferred. Qualified candidates must be committed to our wellness philosophy and Franciscan values and possess an engaging personality.

Dining Attendant

Category: Part-Time

Position Summary The Dining Attendant provides table service to residents, guest, and staff.

Requirements High school diploma or GED preferred. Experience in food service or health care preferred.  Qualified candidates must be committed to our wellness philosophy and Franciscan values and possess an engaging personality.

Station House Server

Category: Full-Time

Position Summary The Server provides table service to residents, guests, and staff in the Station House Restaurant. This position is part time with no late hour requirements.

Resident Assistant

Category: Part-Time

Position Summary Under the direct supervision of the nurse in charge, the Resident Assistant (RA) performs the basic nursing duties to provide for the comfort, safety and hygiene needs of Assisted Living residents. The RA assists AL residents in maintaining and possibly increasing their level of independence whenever possible.

Requirements Experience in the position is recommended. Must be able to work every other weekend and holiday. Qualified candidates must be committed to our wellness philosophy and Franciscan values and possess an engaging personality.

Billing Specialist

We are looking for a Billing Specialist who has 5 or more years of experience with Medicare/Medicaid/Private Insurance in a long term care setting and is committed to providing exceptional customer service to residents, families, and the community in a faith-based senior services environment. Some of the duties of this position includes: account receivables, verification of completed admissions forms, contracts, and discharge of residents, assistance with addressing resident/family billing questions, prepares/delivers bank deposits, files for Medicaid benefits for residents, and maintains the resident trust accounts. Must have experience with Point Click Care.

Groundskeeper

Groundskeepers are required to perform all tasks required to maintain the grounds, gardens, walks and roads in a safe, appealing condition. All personnel are on-call for snow and ice removal in the winter. Two years experience preferred. Must have a valid driver’s license with a safe driving record. Qualified candidates will possess a genuine heart for the aging population and appreciation of a faith based environment.

Housekeeping Aid

Position Summary The housekeeper is responsible for cleaning resident rooms, common areas, and campus facilities in the Health Care Center, Main Building, San Damiano, and other campus buildings as assigned.

Requirements High School Diploma or GED preferred. Previous Health Care experience preferred.

Security Officer

Category: Full Time (2nd Shift)

Position Summary The Security Officer is responsible for the security and safety of all residents, staff and employees. Security Officers monitor all emergency and life safety systems. Security also provides help and contacts for after-hours for Maintenance and other departments.

Requirements High school graduate or equivalent. Experience is preferred, but not required for this position. Regular attendance is an essential part of this job description.

Unit Manager – HRC

Category: Full-Time

The Unit Manager is responsible for assuring quality resident care, maintaining nursing service objectives and standards of nursing practice, in conjunction with the Director of Nursing. The Unit Manager serves as a resource person in both the clinical and management setting. This position also acts as a positive clinical role model and effective change agent to ensure optimum delivery of nursing care and services.

Personal Trainer

Category: Part-Time

The Personal Trainer for the Wellness Center is responsible for coordinating the delivery of wellness programs and services under the direction of the Director of Wellness, based on resident and community needs. The Personal Trainer is responsible for developing and maintaining Wellness Center memberships.

Department: Wellness
Reports to: Wellness Center Manager

  1. Coordinate Wellness Center memberships
  2. Assist in developing community relationships
  3. Assist in the design and implement of wellness programs under the direction of the Wellness Center Manager and the Fitness Supervisor
  4. Responsible for maintaining up-to-date knowledge and complying with policies and procedures of Infection Control, OSHA, Safety, State, and Federal laws. Interact effectively within multi-disciplinary teams.

Maintenance Technician

Department: Maintenance
Reports to: Director of Facilities

  1. Responsible for keeping plumbing systems and fixtures in good working order.
  2. Shall perform maintenance, repair and installation of electrical systems and components.
  3. Shall perform maintenance, repair and installation of HVAC equipment and systems and refrigeration.
  4. Shall perform maintenance, repair and installation of appliances.
  5. Shall perform tasks of general maintenance.

Spiritual Care Associate

Department: Mission Integration
Reports to: Manager of Spiritual Care

  1. Works with the Manager of Spiritual Care to plan events, programs and liturgies for the department, employees and all the residents. Prepares the Liturgical Intercessions for each month.
  2. Visits residents in Assisted Living on a routine basis and checks with nursing supervisors and/or social workers for referrals to residents who may be in need of special care, i.e. as death approaches; documents all visits in the resident’s medical chart at the Health and Rehabilitation Center; and in the proper file in the Spiritual Care Office for Assisted Living and Chaminade residents; monitors, updates and completes assessments for Health and Rehabilitation Center, Assisted Living and Chaminade Hall residents.
  3. Attends the Ethics Committee meetings, Spiritual Care staff meetings and Employee Open Forums.
  4. When scheduled, assists Protestant ministers who come for weekly services in the main chapel or the Health and Rehabilitation Center. Plans and conducts services for the residents in the event a minister is unavailable.
  5. Leads grief groups, Bible study or religious discussion programs as requested of the Spiritual Care department.
  6. Assists Manager of Spiritual Care with Bereavement Teams, funeral planning, and funerals as needed. Keeps a computerized ongoing record of deaths at St. Leonard.
  7. Demonstrates the knowledge and skills necessary to provide care appropriate to the ages of the residents served.
  8. Prioritizes work in order to be available to the residents for one-on-one conferences.
  9. Satisfies all education requirements mandated by St. Leonard and external accrediting and regulatory agencies.
